1. New and existing policyholders who pay 1 year premium within the promo period from July 1, 2003 to July 31, 2004 are qualified in the raffle. Existing policyholders who have not yet paid their 1 year premium can qualify in the raffle if they pay for the balance of their 1 year premium within the promo period.
2. The raffle is open to every policyholder who have paid one (1) year premium.
3. The raffle ticket entitlement of every qualified policyholder is as follows:
Plan A - 6 raffle tickets
Plan B - 5 raffle tickets
Plan C - 4 raffle tickets
Plan D - 3 raffle tickets
Plan E - 2 raffle tickets
Plan F - 2 raffle tickets
4. The promo period starts July 1, 2003. Deadline is on July 31, 2004.
5. The following prizes are at stake:
Two (2) 1st prizes: 21 Polytron Color TV
Two (2) 2nd prizes: Mini-Component with VCD and Sub-Woofer
Three (3) 3rd prizes: 2-Burner Gas Stove with LPG Tank
6. Winners will be drawn during the 3rd PAKISAMA Mutual National Convention on August 2004.
7. A policyholder can only win once. The prize with the higher value will be awarded.
8. The Kadamay of the winning policyholder will get the same prize.
9. Winners will be notified through telegram as to where they can claim the prize. Prizes not claimed within 60 days of notification will be forfeited in favor of the PAKISAMA Mutual.
10. Winners will be asked to present their winning raffle ticket bearing the corresponding serial number, plus any of the following: SSS ID, Pag-IBIG ID, passport, driver's license, PAKISAMA ID, or any valid ID with picture.
11. The winner will shoulder the cost of freight and delivery, and tax.
